Health coaches connect with members following receipt of screening results.
Personal Health Profile (PHP) outreach
Health Coaching begins once a member completes their Health Risk Assessment (HRA) and biometric screening, generating an extensive report that compares an individual’s results to national standards.
Gaps in Care outreach
Approximately 3 to 6 months from program start to specifically address members who may not be using preventive services and to follow up and ensure they are getting necessary screenings.
